Participant Waiver

Release of Liability

• I am the parent or legal guardian of the registered athlete.
• I understand volleyball involves physical activity with inherent risk of injury.
• I voluntarily accept these risks on behalf of my child.
• I release Shmack It Volleyball, its coaches, and staff from liability for injuries sustained during normal participation.
• My child is physically fit and cleared to participate in athletic activity.
• I consent to emergency medical care if needed and accept financial responsibility for such care.

Cancellation policy: No refunds within 48 hours of a scheduled session. Weather cancellations will be rescheduled.
